State of Michigan Controlled Substance Inventory Due by June 30

REMINDER: State of Michigan Controlled Substance Annual Inventory is due by JUNE 30.

If you have a controlled substance research license with the State of Michigan you are required to submit an annual inventory to the State of Michigan between April 1 and June 30 each year.

Step 1: Complete the inventory

  • A sample inventory form can be found on the EHS website. There is no specific form that must be used.  The form can be used as is or modified for your use. 
  • Instructions on completing the form can be found on the EHS website.
  • If there are no controlled substances in stock at the registered location at the time of the annual inventory, the licensee/registrant must still report a zero inventory.

Step 2: Send a copy of your annual inventory to:

Step 3: Keep a copy of your annual inventory on hand to meet the requirements of the federal regulations for a biennial inventory (see the regulation: 21 CFR 1304.11).
